A Michigan OSHA investigation found a total of six violations by Fair Salvage in the tire explosion death of Ethan Gage. The accident happened back in February. Violations include not providing proper training, not providing proper safety equipment and not reporting the death to OSHA within 8 hours. They were fined $38,000.
